Fertility Preservation

Indications

In Women:

  • Physiological aging
  • Premature ovarian aging
  • Prior to chemo – /radiation therapies for cancer
  • Prior to other medical treatments which have the potential of harming ovaries
  • Prior to ovarian surgery

In Men:

  • Prior to chemo – /radiation therapies for cancer
  • Prior to other medical treatments which have the potential of affecting fertility
  • Prior to prostate or testicular surgery
  • Before a vasectomy
  • In high-risk occupation, exposure to toxic materials or radiation

Procedure

In Women:

Oocyte cryopreservation is not an established medical treatment. Women who may be considering the procedure must be well informed about the process involved and the clinical outcomes that reasonably can be expected.

In order to stimulate multiple egg growth, the patient will be given, by injection, hormonal medications. When, on the basis of hormone measurements and ultrasound scans, optimal maturity of eggs has been achieved, mature eggs are retrieved from the ovaries, under ultrasound guidance. Physical damage may result to individual eggs due to the stress of freezing and thawing. Oocytes that survive thawing have the potential to establish a pregnancy after being fertilized with sperm, through an IVF procedure.

In Men:

Cryopreserving sperm is a much simpler procedure than freezing eggs. Several samples of sperm might be collected and frozen, in order to increase the chances of a successful fertilization procedure.

Sperm can be stored for many years. If the sperm is normal, it might be use for artificial insemination of the female partner. If the samples have low quality, IVF might be the best treatment to choose.
